Data Governance talks to WHAT data we collect from your practice and WHY. It also specifies the data we DON’T collect, and HOW and WHO we share your reports with.
We believe that these aspects need to be communicated in a clear and transparent way, so you have the relevant information you need to grant informed consent to your participation in our analytics programs.
Every program conducted by PracticeProfiles will have its own consent form that clearly outlays the terms and conditions of your practice’s involvement.
For our most expansive program in terms of scope of data collected, we collect data that allows us to report back to you on quality improvement metrics.
The types of quality improvement metrics we report on include (but are not limited to):
– Compliance with clinical guidelines
– Quality of medical record keeping
– Population health outcomes
We never collect personal information that can identify a patient.
PraxMax Clinic allows your practice to find patients that can benefit from clinical interventions and recalls (per the reports we generate for you) by linking to your clinical database and reading a reference specific to each patient (that was originally sent with the data) that re-identifies the patient within your firewalls only.
We collect identities of practitioners (if you consent) in order for us to report back to you for your own internal use. In this instance, you must get their consent to such collection (usually in employment/contractor terms) and give them a copy of our privacy policy. Under these circumstances, we will never share the identities of your practitioners with any third-party to your practice.
If you give consent for us to do so, we can share your un-identified data with third parties. Under these circumstances, we will never share your data with another medical practice.
